Your child's health history

is an important part

of keeping your child

safe at SMA. Forms need to be updated every school year as your child grows and his or her medical needs change.

Nothing is more important to us than the safety of your children.  It is imperative that each child in the building is properly accounted for at all times. Running late or have an appointment?Contact our Main Office to let us know! 

SMA provides technology resources to students to promote educational excellence.  Each student

and guardian must sign

an agreement of

proper internet and Chromebook usage.

Student Transportation forms vary for each district, but must be filled out each year, usually by April 1st of the previous school year.  Please visit the Transportation Forms page for more information.
